1. Who we are

2. What we collect
- Name, age or date of birth, gender, mobile number, an alternate number, and email if you give one.
- Address, village or town, taluka, district, state and PIN code.
- Scheme details: which scheme, whether you hold an Ayushman card, and the card number if you choose to give it.
- The treatment needed, the department, and a short description of the medical problem.
- Documents you upload.
- Technical records: IP address, browser information and the time you gave consent.
3. What we deliberately do not collect
We do not ask for Aadhaar numbers, bank account details, income information, biometric data or a full medical history. There are no fields for these anywhere in the form.
4. Why we collect it
Only to guide you on health schemes, check your paperwork, coordinate with hospitals and contact you. We do not use it for advertising and we do not sell it.
5. Who can see it
- Only authorised staff, after signing in, and only as much as their role requires.
- Counsellors can only open cases assigned to them.
- Every time a document is opened or downloaded it is recorded in an audit log.
- We do not share your information with third parties without your agreement or a legal obligation.
6. How documents are stored
Uploaded documents are stored outside the public part of the website. There is no public URL for them; permission is checked each time a file is opened, and the stored filename is randomly generated.
7. How long we keep it
For as long as the case is active, and afterwards according to our records retention policy. The agreed retention period is: [RETENTION PERIOD]. After that, records are archived or deleted.
8. Your choices
To see what we hold about you, correct it, withdraw your consent or ask us to delete a record, contact us on the number above. If you withdraw consent we will not be able to continue assisting with your case.
9. Security
The site uses HTTPS, passwords are stored only as hashes, sensitive actions are logged, and access is limited by role. Even so, no system can be claimed to be completely secure.
10. Changes to this notice
This notice is versioned. We record which version you consented to at the time you submitted your request.
