What is this scheme?
The Mahatma Jyotirao Phule Jan Arogya Yojana is a health scheme of the Government of Maharashtra. Eligible beneficiaries may receive cashless treatment at hospitals empanelled under the scheme, for procedures included in the scheme. Which procedures are covered, who qualifies and the extent of cover are all decided by the State and are revised from time to time.
What is usually needed
Ayushman card or health scheme card

A government-issued photo ID.
Ration card
Required by some schemes.

Doctor's prescription
Current treatment being followed.
