Documents you may need
These are the documents usually asked for. Exactly which ones apply depends on the scheme and the hospital.
Ayushman card or health scheme card
If you have one. If not, we will explain the options.
Photo identity proof of the patient
A government-issued photo ID.
Ration card
Required by some schemes.
Medical reports
Reports and investigations relating to the diagnosis.
Doctor's prescription
Current treatment being followed.
Hospital estimate
If the hospital has given you one.
Sending us your documents
- You can upload PDF, JPG or PNG files through the registration form.
- If you photograph a document, make sure the whole page and the text are clearly visible.
- Apply even if you do not have everything yet — you can provide the rest later.
- Your documents are stored outside the public part of the website and are visible only to authorised staff.
Please do not send medical reports by email or WhatsApp. Use the registration form, or bring them to the office.
